Hosanna’s Haven accepts referrals from:

•  Hospitals and discharge planning teams

•  Case managers and social service agencies

•  Veteran service organizations

•  Community based partners

All referrals are reviewed to confirm program alignment and suitability.

Individuals referred to Hosanna’s Haven should:

•  Be adults able to participate in a shared, structured living environment
•  Not require emergency shelter or clinical care
•  Be able to manage activities of daily living independently
•  Be willing to follow house guidelines and program expectations

This criteria helps maintain a safe and stable environment for all residents.

Referral partners may initiate placement by contacting Hosanna’s Haven through our inquiry form or designated referral contact.

Referrals typically include:
•  Basic demographic information
•  Current housing status and transition need
•  Relevant case management or discharge details
•  Known supports or service connections

All placements are coordinated through scheduled communication. In person visits are not available.
